Wyrażam zgodę na publikowanie mojego wizerunku, utrwalonego podczas Tatrzańskiego Festiwalu Biegowego Tatra SkyMarathon przeprowadzonego w 2025 r., przez Klub Wysokogórski w Zakopanem z siedzibą w Zakopanem oraz przez sponsorów tego Festiwalu, w szczególności przez Malavi Sp. z o.o. sp.k. w celu zrelacjonowania przebiegu, promocji Festiwalu i jego sponsorów na przyszłość w tym  spersonalizowanych wiadomości e-mail dotyczących promocji, nowości, rekomendowanych produktów oraz informacji handlowych od Malavi Sp. z o.o. Sp.K. Zgoda obejmuje publikację we wszelkich środkach masowego przekazu w tym, w prasie, telewizji oraz sieci Internet.

I consent to the publication of my image, recorded during the Festival Tatra SkyMarathon held in 2025, by the Klub Wysokogórski w Zakopanem, as well as by the sponsors of this Festival, in particular by Malavi Sp. z o.o. sp.k., for the purpose of reporting on the event, promoting the Festival and its sponsors in the future, including personalized email messages regarding promotions, news, recommended products, and commercial information from Malavi Sp. z o.o. sp.k. This consent includes publication in all mass media, including press, television, and the Internet.    
Oświadczam, że zapoznałem się i akceptuję regulamin zawodów oraz zamieszczoną w nim klauzulę informacyjną RODO. Zobowiązuję się do przestrzegania wszystkich tych zasad bez żadnego wyjątku, a także instrukcji i decyzji osób funkcyjnych. Zostałem poinformowany, jestem świadom ryzyka i niebezpieczeństw mogących zaistnieć podczas udziału w Tatrzańskim Festiwalu Biegowym, Tatra SkyMarathon, Orava Trail, Tatra Trail, Mini Tatra Sky, w dniu 26.07.2025 r. , w szczególności ryzyka wypadku, odniesienia obrażeń ciała i urazów fizycznych (w tym śmierci) a także szkód i strat o charakterze majątkowych.

I declare that I have read and accept the competition regulations, including the GDPR information clause contained therein. I undertake to comply with all these rules without exception, as well as the instructions and decisions of the event officials. I have been informed, I am aware of the risks and dangers that may occur during participation in the Tatra SkyMarathon Festival, Tatra SkyMarathon, Orava Trail, Tatra Trail, Mini Tatra Sky on July 26, 2025, in particular the risk of accidents, bodily injuries, and physical harm (including death), as well as material damages and financial losses. 
Oświadczam, że znam swój stan zdrowia i jestem zdolny do udziału w zawodach w biegach górskich typu skyruning. Potwierdzam, że nie występują w przypadku mojej osoby żadne przeciwwskazania zdrowotne do udziału w Tatrzańskim Festiwalu Biegowym.

I declare that I am aware of my health condition and that I am fit to participate in mountain running competitions of the skyrunning type. I confirm that there are no health contraindications preventing me from taking part in the Tatra SkyMarathon Festival.
